Here's all you have to do...answer the questions on the scavenger hunt entry form and return it to us. All returned forms with 9-12 correct answers will be awarded 25 reward points. Entries with all 12 questions answered correctly will be entered into a drawing to win 50 additional reward points! Only one entry per patient will be accepted. All forms must be returned by April 30th to qualify for points.
